SECTION 926.250 REQUESTS FOR ACCESS TO RECORDS


 

Section 926.250  Requests for Access to Records

 

Requests under the FOIA for access to public records maintained by the Department shall be made in writing and submitted to:  FOIA Officer, Department of Human Rights, 100 West Randolph Street, Suite 10-100, Chicago, Illinois 60601.  Each request should be clearly marked "ATTN:  FOIA Request."  If an individual is unable, because of disability, to prepare a written request, the request may be submitted by telephone to the FOIA Officer.  The request must include the requester's full name, address and telephone number; a description of the specific records sought; and a statement as to whether the request is for inspection, for copying, or both.
