Section 200.426  Form of Papers

All papers filed or submitted to the Department in a contested case shall be typewritten on 8½ by 11 inch white paper.  The first page of each document shall set forth the name of each of the respondents and the file number assigned to the case by the Department.  All pleadings must be signed by the party filing the same or authorized representative or attorney, and shall contain the party's business address and telephone number.  A copy of any pleading shall be filed with the Administrative Law Judge, and the original served upon the attorney of record of the Department.
