Section 116.80  Documentation

The applicant must provide the following official documentation when applicable:

a)         Marriage Certificate

b)         Divorce Decree

c)         Birth Certificate

d)         Adoption Decree

e)         Letters of Guardianship

f)          Death Certificate or Report of Casualty

g)         DD 214 or Discharge

h)         Proof of Disability (Statement from Department of Defense or Veterans Administration)
