AMENDMENT TO HOUSE BILL 4396

2    AMENDMENT NO. ______. Amend House Bill 4396 by replacing
3everything after the enacting clause with the following:
 
4    "Section 5. The Unified Code of Corrections is amended by
5changing Section 3-5-3 as follows:
 
6    (730 ILCS 5/3-5-3)  (from Ch. 38, par. 1003-5-3)
7    Sec. 3-5-3. Annual and other Reports.
8    (a) The Director shall make an annual report to the
9Governor and General Assembly concerning persons committed to
10the Department, its institutions, facilities and programs, of
11all moneys expended and received, and on what accounts expended
12and received. The report shall include the ethnic and racial
13background data, not identifiable to an individual, of all
14persons committed to the Department, its institutions,
15facilities, and programs.
16    (b) (Blank).

1    (c) The Director may require such reports from division
2administrators, chief administrative officers and other
3personnel as he deems necessary for the administration of the
4Department.
5    (d) (Blank).
6    (e) The Director shall make quarterly reports to the
7Governor and the General Assembly concerning each incident of
8assault by a committed person toward Department of Corrections
9staff. These reports shall specify the facility where the
10incident occurred, the classification of offender that
11committed the assault, and the nature of the injury to the
12staff member, if any.
13    (f) The Director shall make quarterly reports to the
14Governor and the General Assembly concerning the use of force
15incidents administered by Department of Corrections staff on
16persons committed to the Department. The reports shall specify
17the facility where the incident occurred, the classification of
18the offender that was the recipient of the use of force, and
19the nature of the injury, if any.
