HOUSE RESOLUTION

 
2    WHEREAS, The members of the Illinois House of
3representatives wish to congratulate Sunrise of Gurnee on
4earning the Gold-Excellence in Quality Award; and
 
5    WHEREAS, The American Health Care Association National
6Center for Assisted Living National Quality Award Program
7provides a pathway for providers of long-term and post-acute
8care services to journey towards performance excellence; the
9program is based on the core values and criteria of the
10Baldrige Performance Excellence Program; and
 
11    WHEREAS, The three progressive levels of awards are the
12Bronze-Commitment to Quality, the Silver-Achievement in
13Quality, and the Gold-Excellence in Quality; each level has its
14own distinct rigors and requirements for quality and
15performance excellence; and
 
16    WHEREAS, Since earning the Bronze Quality Award in 2015,
17Sunrise of Gurnee has achieved two important milestones; the
18community was one of the first four Sunrise communities to
19achieve Silver in 2016, and one of the first two assisted
20living communities nationwide to achieve Gold; and
 
21    WHEREAS, Sunrise of Gurnee has increased their team member

1retention throughout their quality journey; since 2013, they
2have demonstrated their commitment to staff development
3through the promotion of many team members within Sunrise
4Senior Living; and
 
5    WHEREAS, Sunrise of Gurnee implemented a buddy system for
6team members, which has increased engagement and morale,
7contributing to the overall positive culture of its community;
8and
 
9    WHEREAS, As a result of its consistent, quality care,
10Sunrise of Gurnee has achieved a reduction in its hospital
11readmission rate; and
 
12    WHEREAS, Preparing the Gold application helped Sunrise of
13Gurnee's community embrace a systematic, evidence-based
14approach to person-centered care; the collaboration and
15exchange of critical information across departments enabled it
16to identify ways to help enhance quality outcomes; gathering
17the various success stories for the application process
18strengthened the team's pride in the achievement of differences
19they have made in the lives of its residents; therefore, be it
 
20    RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE
21HUNDREDTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we
22congratulate Sunrise of Gurnee on receiving the

1Gold-Excellence in Quality Award given by the American Health
2Care Association National Center for Assisted Living Quality
3Award Program; and be it further
 
4    RESOLVED, That a suitable copy of this resolution be
5presented to Sunrise of Gurnee as an expression of our esteem
6and respect.
