AMENDMENT TO HOUSE BILL 3811 

2    AMENDMENT NO. ______. Amend House Bill 3811 as follows:
 
3on page 4, line 18, by replacing "120" with "60"; and
 
4on page 4, line 23, by replacing "120" with "60"; and
 
5on page 4, line 24, after "guardians", by inserting "or when 
6guardianship services are paid for by a hospital licensed 
7under the Hospital Licensing Act or the University of Illinois 
8Hospital Act"; and
 
9on page 5, line 5, by deleting "or entity"; and
 
10on page 6, by replacing lines 7 through 24 with the following:
11        "(1)(i) The private professional guardian will 
12    personally meet with and assess the respondent before or 
13    as soon as feasible after the appointment; (ii) evaluate

1    what is the least restrictive form of guardianship that is 
2    appropriate; and (iii) evaluate options regarding the 
3    respondent's living arrangements including 
4    community-based, least restrictive living settings that 
5    incorporate the respondent's values, preferences, and 
6    known wishes.".
