Section 105.408  Petition Content Requirements

In addition to the requirements of 35 Ill. Adm. Code 101.Subpart C the petition must contain:

a)         The Agency's final decision;

b)         A statement specifying the date of service of the Agency's final decision; and

c)         A statement specifying the grounds of appeal.
