Section 123.220  Application Process

a)         An application may be made for the program using any of the methods established at 89 Ill. Adm. Code 110.5.

b)         The application will meet all requirements found at 89 Ill. Adm. Code 110.10.

c)         Applicants are obligated to provide truthful and accurate information for determining eligibility and to report promptly to the Department any change in information provided on the application.

d)         The application review process shall comply with 89 Ill. Adm. Code 110.20 and 110.40.
