| 4 | on page 3, line 12, after "year.", by inserting "The reduction | ||||||
| 5 | for Medicare premiums shall be made only upon proof of payment | ||||||
| 6 | of Medicare premiums by the taxpayer; that proof shall include | ||||||
| 7 | dates and amounts and may take the form of bill payment stubs | ||||||
| 8 | such as the CMS-500 form.".
