1 AN ACT concerning driver's licenses, amending named Acts.
2 Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
4 Section 5. The State Finance Act is amended by adding
5 Section 5.490 as follows:
6 (30 ILCS 105/5.490 new)
7 Sec. 5.490. The Prevent Blindness Fund.
8 Section 10. The Illinois Vehicle Code is amended by
9 changing Section 6-106 as follows:
10 (625 ILCS 5/6-106) (from Ch. 95 1/2, par. 6-106)
11 Sec. 6-106. Application for license or instruction
12 permit.
13 (a) Every application for any permit or license
14 authorized to be issued under this Act shall be made upon a
15 form furnished by the Secretary of State. Every application
16 shall be accompanied by the proper fee and payment of such
17 fee shall entitle the applicant to not more than 3 attempts
18 to pass the examination within a period of 1 year after the
19 date of application.
20 (b) Every application shall state the name, social
21 security number, zip code, date of birth, sex, and residence
22 address of the applicant; briefly describe the applicant;
23 state whether the applicant has theretofore been licensed as
24 a driver, and, if so, when and by what state or country, and
25 whether any such license has ever been cancelled, suspended,
26 revoked or refused, and, if so, the date and reason for such
27 cancellation, suspension, revocation or refusal; shall
28 include an affirmation by the applicant that all information
29 set forth is true and correct; and shall bear the applicant's

1 signature. The application form may also require the
2 statement of such additional relevant information as the
3 Secretary of State shall deem necessary to determine the
4 applicant's competency and eligibility. The Secretary of
5 State may in his discretion substitute a federal tax number
6 in lieu of a social security number, or he may instead assign
7 an additional distinctive number in lieu thereof, where an
8 applicant is prohibited by bona fide religious convictions
9 from applying or is exempt from applying for a social
10 security number. The Secretary of State shall, however,
11 determine which religious orders or sects have such bona fide
12 religious convictions. The Secretary of State may, in his
13 discretion, by rule or regulation, provide that an
14 application for a drivers license or permit may include a
15 suitable photograph of the applicant in the form prescribed
16 by the Secretary, and he may further provide that each
17 drivers license shall include a photograph of the driver. The
18 Secretary of State may utilize a photograph process or system
19 most suitable to deter alteration or improper reproduction of
20 a drivers license and to prevent substitution of another
21 photo thereon.
22 (c) The application form shall include a notice to the
23 applicant of the registration obligations of sex offenders
24 under the Sex Offender Registration Act. The notice shall be
25 provided in a form and manner prescribed by the Secretary of
26 State. For purposes of this subsection (c), "sex offender"
27 has the meaning ascribed to it in Section 2 of the Sex
28 Offender Registration Act.
29 (d) The application form shall include a provision
30 indicating that if the applicant wishes to contribute to the
31 Prevent Blindness Fund he or she may do so by stating the
32 amount of the contribution (not less than $1) on the
33 application form. A statement providing an explanation of the
34 purpose of the contribution shall be included with the

2 (e) The Prevent Blindness Fund is created as a special
3 fund in the State treasury. All money in the Prevent
4 Blindness Fund shall be paid, subject to appropriation by the
5 General Assembly and approval by the Secretary, as grants to
6 the Illinois based Prevent Blindness America, a
7 not-for-profit organization, for the purpose of preventing
8 blindness and preserving the sight of residents of this
9 State.

11 Section 99. Effective date. This Act takes effect
12 January 1, 2000.
