1 AMENDMENT TO HOUSE BILL 2644
2 AMENDMENT NO. . Amend House Bill 2644 by replacing
3 everything after the enacting clause with the following:
4 "Section 5. The Dietetic and Nutrition Services Practice
5 Act is amended by changing Section 85 as follows:
6 (225 ILCS 30/85) (from Ch. 111, par. 8401-85)
7 Sec. 85. Fees. The Department shall provide by rule for
8 a schedule of fees for the administration and enforcement of
9 this Act, including but not limited to original licensure,
10 renewal, and restoration. The fees shall be nonrefundable.
11 All fees collected under this Act shall be deposited into
12 the General Professions Dedicated Fund and shall be
13 appropriated to the Department for the ordinary and
14 contingent expenses of the Department in the administration
15 of this Act. The following fees shall be imposed by the
16 Department and are not refundable:
17 (a) A fee of $100 must accompany an application for a
18 license under this Act.
19 (b) In addition, applicants for any examination as a
20 dietitian shall be required to pay, either to the Department
21 or to the designated testing service, a fee covering the cost
22 of providing the examination. Failure to appear for the

1 examination on the scheduled date, at the time and place
2 specified, after the applicant's application for the
3 examination has been received and acknowledged by the
4 Department or the designated testing service, shall result in
5 the forfeiture of the examination fee.
6 (c) The fee for the renewal of a license as a dietitian
7 or nutrition counselor shall be $50 per year.
8 (d) The fee for application for a license for a
9 dietitian or nutrition counselor licensed under the laws of
10 another jurisdiction is $200.
11 (e) The fee for the restoration of a license other than
12 from inactive status is $25 plus payment of all lapsed
13 renewal fees.
14 (f) The fee for the issuance of a duplicate license, for
15 the issuance of a replacement registration for a license that
16 has been lost or destroyed, or for the issuance of a license
17 with a change of name or address other than during the
18 renewal period is $25. No fee is required for name and
19 address changed on Department records where a duplicate
20 registration is not issued.
21 (g) The fee for certification of a license for any
22 purpose is $25.
23 (h) The fee to have the scoring of an examination
24 administered by the Department reviewed and verified is $25
25 plus any fees charged by the applicable testing service.
26 (i) The fee for a wall certificate showing a license
27 shall be the actual cost of producing the certificate.
28 (j) The fee for a roster of licensees as dietitians or
29 nutrition counselors in this State shall be the actual cost
30 of producing the roster.
31 (k) The fee for initial approval of a continuing
32 education sponsor shall be $500. The fee for renewal of
33 sponsor approval shall be $250 every 2 years. State colleges,
34 universities, and State agencies located in Illinois are

1 exempt from payment of this fee.

3 Section 10. The Environmental Health Practitioner
4 Licensing Act is amended by changing Section 28 as follows:
5 (225 ILCS 37/28)
6 Sec. 28. Fees. The Department shall provide by rule for
7 a schedule of fees for the administration and enforcement of
8 this Act, including but not limited to original licensure,
9 renewal, and restoration. The fees shall be nonrefundable.
10 The fees imposed under this Act are as follows and are
11 nonrefundable:
12 (1) The fee for application for an environmental health
13 practitioner license is $100.
14 (2) Applicants for examination shall pay, either to the
15 Department or to the designated testing service, a fee
16 covering the cost of providing the examination.
17 (3) The fee for renewal of a license is $110 per year.
18 (4) The fee for reinstatement of a license that has
19 expired for less than 5 years is $20, plus payment of all
20 unpaid fees for every year that has lapsed.
21 (5) The fee for the restoration of a license, other than
22 from inactive status, is $10 plus payment of all lapsed
23 renewal fees.
24 (6) The fee for the restoration of a license that has
25 expired for more than 5 years is $300.
26 (7) The fee for issuance of a duplicate license, a
27 replacement license that has been lost or destroyed, or a
28 license with a change of name or address, other than during
29 the renewal period, is $20. No fee is required for name and
30 address changes on Department records when no duplicate
31 license is issued.
32 (8) The fee for certification of a license for any

1 purpose is $20.
2 (9) The fee for a wall certificate showing the granting
3 of a license shall be the actual cost of producing the
4 certificate.
5 (10) The fee for a roster of persons licensed as
6 environmental health practitioners in this State shall be the
7 actual cost of production of the roster.
8 (11) The fee for a sponsor of continuing education shall
9 be set by rule.

11 Section 15. The Funeral Directors and Embalmers
12 Licensing Code is amended by changing Section 15-65 as
13 follows:

15 Sec. 15-65. Fees. The Department shall provide by rule
16 for a schedule of fees for the administration and enforcement
17 of this Act, including but not limited to original licensure,
18 renewal, and restoration. The fees shall be nonrefundable.
19 All fees collected under this Act shall be deposited into
20 the General Professions Dedicated Fund and shall be
21 appropriated to the Department for the ordinary and
22 contingent expenses of the Department in the administration
23 of this Act. The following fees shall be paid to the
24 Department for the licensing and registration functions
25 performed by the Department under this Code:
26 (A) License of funeral director and embalmer trainee.
27 (1) The fee to be paid by an applicant for a
28 license as a funeral director and embalmer trainee is
29 $50.
30 (2) The fee to be paid upon the renewal of a
31 license as a funeral director and embalmer trainee is
32 $100.

1 (B) License of funeral director and embalmer or funeral
2 director.
3 (1) The fee to be paid by an applicant for a
4 license as a funeral director and embalmer is $100. The
5 Department may provide for examination fees to be paid
6 directly to a designated testing service.
7 (2) The fee to be paid by an applicant for a
8 license as a funeral director and embalmer licensed under
9 the laws of another State or territory of the United
10 States or of a foreign country or province is $200.
11 (3) The fee to be paid upon the renewal of a
12 license as a funeral director and embalmer is $100; and
13 $50 for a funeral director.
14 (4) The fee to be paid for the reinstatement of a
15 license as a funeral director and embalmer or funeral
16 director that has been expired for not more than 5 years
17 is $10 plus all lapsed renewal fees.
18 (5) The fee to be paid for the restoration of a
19 license as a funeral director and embalmer that has been
20 expired for more than 5 years is $260; and $130 for a
21 funeral director.
22 (6) The fee to place a license on inactive status
23 is $20.
24 (C) General fees.
25 (1) The fee to be paid for the issuance of a
26 duplicate license, for the issuance of a replacement
27 license for a license which has been lost or destroyed or
28 for the issuance of a license with a change of name or
29 address other than during the renewal period is $10.
30 (2) The fee to be paid for a certification of a
31 licensee's record for any purpose is $10.
32 (3) The fee to be paid to have the scoring of an
33 examination administered by the Department reviewed and
34 verified is $10.

1 (4) The fee to be paid by a licensee for a wall
2 certificate showing the license shall be the actual cost
3 of producing the certificate.
4 (5) The fee to be paid for a roster of persons
5 licensed as funeral directors, funeral directors and
6 embalmers, or funeral director and embalmer trainees in
7 this State shall be the actual cost of producing the
8 roster.
9 (6) The fee to be paid to the Department for a
10 printed copy of this Code and of the rules and
11 regulations promulgated for the administration of this
12 Code is $1.

14 Section 20. The Illinois Physical Therapy Act is amended
15 by changing Section 16.1 as follows:

17 Sec. 16.1. Fees. The Department shall provide by rule a
18 schedule of fees for the administration and enforcement of
19 this Act, including but not limited to original licensure,
20 renewal, and restoration. The fees shall be nonrefundable.
21 (a) The application fee for a license is $100.
22 (b) Applicants for any examination shall be required to
23 pay, either to the Department or to the designated testing
24 service, a fee covering the cost of determining an
25 applicant's eligibility and providing the examination.
26 Failure to appear for the examination on the scheduled date,
27 at the time and place specified, after the applicant's
28 application for examination has been received and
29 acknowledged by the Department or the designated testing
30 service, shall result in the forfeiture of the examination
31 fee.
32 (c) The fee for the renewal of a license shall be

1 calculated at the rate of $30 per year.
2 (d) The fee for the restoration of a license other than
3 from inactive status is $10 plus payment of all lapsed
4 renewal fees.
5 (e) The fee to be paid for the issuance of a duplicate
6 license, for the issuance of a replacement license, for a
7 replacement license for a license that has been lost or
8 destroyed, or for the issuance of a license with a change of
9 name or address other than during the renewal period is $20.
10 (f) The fee for a certification of a license for any
11 purpose is $20.
12 (g) The fee to have the scoring of an examination
13 administered by the Department reviewed and verified is $20
14 plus any fee charged by the applicable testing service.
15 (h) The fee for a wall certificate evidencing licensing
16 shall be the actual cost of producing that certificate.
17 (i) The fee for a roster of persons licensed as physical
18 therapists or physical therapists assistants in this State
19 shall be the actual cost of producing that roster.

21 Section 25. The Respiratory Care Practice Act is amended
22 by changing Section 75 as follows:

24 Sec. 75. Fees. The Department shall provide by rule
25 for a schedule of fees for the administration and enforcement
26 of this Act, including but not limited to original licensure,
27 renewal, and restoration. The fees shall be nonrefundable.
28 The fees imposed under this Act are as follows and are
29 nonrefundable:
30 (a) The fee for application for a respiratory care
31 practitioner license is $100.
32 (b) The fee for application for a temporary license is

1 $50 per year.
2 (c) Applicants for examination shall pay, either to the
3 Department or to the designated testing service, a fee
4 covering the cost of providing the examination.
5 (d) The fee for the renewal of a license is $60 per
6 year.
7 (e) The fee for the reinstatement of a license that has
8 been expired for less than 5 years is $20, plus payment of
9 all unpaid fees for every year that has lapsed.
10 (f) The fee for the restoration of a license that has
11 been expired for more than 5 years is $300.
12 (g) The fee for the issuance of a duplicate license, the
13 issuance of a replacement for a license that has been lost or
14 destroyed, or the issuance of a license with a change of name
15 or address, other than during the renewal period, is $20. No
16 fee is required for name and address changes on Department
17 records when no duplicate registration is issued.
18 (h) The fee for the certification of a license for any
19 purpose is $20.
20 (i) The fee for rescoring an examination is the cost to
21 the Department of rescoring the examination, plus any fees
22 charged by the applicable testing service to have the
23 examination rescored.
24 (j) The fee for a wall certificate showing the license
25 is the actual cost of producing the certificate.
26 (k) The fee for a roster of persons licensed as
27 respiratory care practitioners in this State is the actual
28 cost of producing the roster.
29 (l) The fee for copies of a license shall be the actual
30 cost of producing the copies.
31 (m) The fee for application for a license by a
32 respiratory care practitioner registered or licensed under
33 the laws of another jurisdiction is $100.
34 All of the fees collected under this Act shall be

1 deposited into the General Professions Dedicated Fund. The
2 monies deposited into the General Professions Dedicated Fund
3 shall be used by the Department, as appropriated, for the
4 ordinary and contingent expenses of the Department. Monies
5 in the General Professions Dedicated Fund may be invested and
6 reinvested, with all earnings received from investments to be
7 deposited into that Fund and used for the same purposes as
8 fees deposited in that Fund.

10 Section 30. The Veterinary Medicine and Surgery Practice
11 Act of 1994 is amended by changing Section 14 as follows:

13 Sec. 14. Fees. The Department shall provide by rule for
14 a schedule of fees for the administration and enforcement of
15 this Act, including but not limited to original licensure,
16 renewal, and restoration. The fees shall be nonrefundable.
17 All fees collected under this Act shall be deposited into
18 the General Professions Dedicated Fund and shall be
19 appropriated to the Department for the ordinary and
20 contingent expenses of the Department in the administration
21 of this Act. The fees under this Act are as follows and are
22 not refundable:
23 (1) The fee for application for a veterinary license is
24 $100.
25 (2) The fee for application for a veterinary technician
26 certificate is $50.
27 (3) In addition to the application fee, applicants for
28 the examination shall be required to pay, either to the
29 Department or to the designated testing service, a fee
30 covering the cost of determining an applicant's eligibility
31 and providing the examination. Failure to appear for the
32 examination on the scheduled date, at the time and place

1 specified, after the applicant's application and fee for
2 examination has been received and acknowledged by the
3 Department or the designated testing service, shall result in
4 the forfeiture of the fee.
5 (4) The fee for the renewal of a veterinary license

7 (5) The fee for the renewal of a veterinary technician
8 certificate is $25 per year.
9 (6) The fee for the reinstatement of a license or

16 technician certificate that has expired for more than 5 years
17 is $150.
18 (9) The fee for the issuance of a duplicate license, for
19 the issuance of a replacement license that has been lost or
20 destroyed, or for the issuance of a license with a change of
21 name or address, other than during the renewal period, is
22 $20. No fee is required for name and address changes on
23 Department records when no duplicate license is issued.
24 (10) The fee for a certification of a license or
25 certificate for any purpose is $20.
26 (11) The fee to have the scoring of an examination
27 administered by the Department reviewed and verified is $20,
28 plus any fee charged by the testing service.
29 (12) The fee for a wall license shall be the actual cost
30 of producing such license.
31 (13) The fee for a roster of persons licensed as
32 veterinarians or veterinary technicians shall be the actual
33 cost of producing the roster.
34 (14) The fee for application for a license by a

1 veterinarian registered or licensed under the laws of another
2 jurisdiction is $100.
3 (15) The fee for application for a certificate by a
4 veterinary technician registered or licensed under the laws
5 of another jurisdiction is $50.

7 Section 35. The Interior Design Profession Title Act is
8 amended by changing Sections 11 and 30 as follows:

10 Sec. 11. Fees. The Department shall provide by rule for
11 a schedule of fees for the administration and enforcement of
12 this Act, including but not limited to original licensure,
13 renewal, and restoration. The fees shall be nonrefundable.
14 All fees collected under this Act shall be deposited into
15 the General Professions Dedicated Fund and shall be
16 appropriated to the Department for the ordinary and
17 contingent expenses of the Department in the administration
18 of this Act. The following fees are not refundable:
19 (a) Registration fees.
20 (1) The fee for application for a certificate of
21 registration as an interior designer or a residential
22 interior designer is $100.
23 (2) The fee for a certificate of registration as an
24 interior designer or residential interior designer for a
25 person who is registered or licensed under the laws of
26 another state, or territory of the United States or of a
27 foreign country or province is $100.
28 (3) The fee for the renewal of a certificate of
29 registration shall be calculated at a rate of $80 per
30 year.
31 (4) The fee for the restoration of a certificate of
32 registration other than from inactive status is $20 plus

1 payment of all lapsed renewal fees.
2 (b) General fees.
3 (1) The fee for the issuance of a duplicate
4 certificate of registration, for the issuance of a
5 replacement certificate of registration, for a
6 certificate of registration which has been lost or
7 destroyed, or for the issuance of a certificate of
8 registration with a change of name or address other than
9 during the renewal period is $20. No fee is required for
10 name and address changes on Department records when no
11 duplicate certificate of registration is issued.
12 (2) The fee for the certification of the
13 registrant's record for any purpose is $20.
14 (3) The fee for rescoring an examination is the
15 cost to the Department of rescoring the examination plus
16 any fees charged by the applicable testing service to
17 have the examination rescored.
18 (4) The fee for a wall certificate showing
19 registration shall be the actual cost of producing such
20 certificate.
21 (5) The fee for a roster of persons registered as
22 interior designers or residential interior designers in
23 this State shall be the actual cost of producing such a
24 roster.

27 Sec. 30. Interior Design Administration and Investigation
28 Fund. All of the fees collected pursuant to this Act shall be
29 deposited into the General Professions Dedicated Fund.
30 On January 1, 2000 the State Comptroller shall transfer
31 the balance of the monies in the Interior Design
32 Administration and Investigation Fund into the General
33 Professions Dedicated Fund. Amounts appropriated for fiscal

1 year 2000 out of the Interior Design Administration and
2 Investigation Fund may be paid out of the General Professions
3 Dedicated Fund.
4 The monies deposited in the General Professions Dedicated
5 Fund may be used for the expenses of the Department in the
6 administration of this Act.
7 All of the fees collected under this Act shall be deposited
8 in the Interior Design Administration and Investigation Fund.
9 The monies deposited in the Interior Design Administration
10 and Investigation Fund shall be appropriated to the
11 Department for expenses of the Department and the Board in
12 the administration of this Act. Monies in the Interior
13 Design Administration and Investigation Fund may be invested
14 and reinvested, with all earnings received from such
15 investment to be deposited in the Interior Design
16 Administration and Investigation Fund and used for the same
17 purposes as fees deposited in the Fund.
18 Moneys from the Fund may also be used for direct and
19 allocable indirect costs related to the public purposes of
20 the Department of Professional Regulation. Moneys in the
21 Fund may be transferred to the Professions Indirect Cost Fund
22 as authorized by Section 61e of the Civil Administrative Code
23 of Illinois.
24 Upon the completion of any audit of the Department as
25 prescribed by the Illinois State Auditing Act that includes
26 an audit of the Interior Design Administration and
27 Investigation Fund, the Department shall make the audit open
28 to inspection by any interested person. The copy of the audit
29 report required to be submitted to the Department by this
30 Section is in addition to copies of audit reports required to
31 be submitted to other State officers and agencies by Section
32 3-14 of the Illinois State Auditing Act.

1 Section 40. The Collection Agency Act is amended by
2 changing Section 8a as follows:
3 (225 ILCS 425/8a) (from Ch. 111, par. 2011a)
4 Sec. 8a. Fees. The Department shall provide by rule for a
5 schedule of fees for the administration and enforcement of
6 this Act, including but not limited to original licensure,
7 renewal, and restoration. The fees shall be nonrefundable.
8 All fees collected under this Act shall be deposited into
9 the General Professions Dedicated Fund and shall be
10 appropriated to the Department for the ordinary and
11 contingent expenses of the Department in the administration
12 of this Act. The following fees shall be paid to the
13 Department for the licensing and registration functions
14 performed by the Department under this Act:
15 (1) The fee to be paid by an applicant for a
16 certificate of registration as a collection agency is
17 $750.
18 (2) The fee to be paid upon the renewal of a
19 certificate of registration as a collection agency is
20 $750 for a 3-year renewal.
21 (3) The fee to be paid for the issuance of a
22 duplicate certificate of registration, for the issuance
23 of a replacement certificate for a certificate which has
24 been lost or destroyed or for the issuance of a
25 certificate with a change of name or address other than
26 during the renewal period is $20.
27 (4) The fee to be paid for a certification of a
28 registrant's record for any purpose is $20.
29 (5) The fee to be paid by a registrant for a wall
30 certificate showing his registration shall be the actual
31 cost of producing such certificate.
32 (6) The fee to be paid for a roster of persons
33 registered as collection agencies in this State shall be

1 the actual cost of producing such a roster.
2 (7) The fee to be paid to the Department for a
3 printed copy of this Act and of the rules and regulations
4 promulgated for the administration of this Act is $1.
5 (8) The fee to be paid by an applicant for a
6 certificate of registration to operate a branch office is
7 $250.
8 (9) The fee to be paid upon the renewal of a
9 certificate of registration to operate a branch office is
10 $150 for a 3-year renewal.
11 All fees and fines collected under this Act shall be
12 deposited into the General Professions Dedicated Fund. All
13 monies in the fund shall be used by the Department of
14 Professional Regulation, as appropriated, for the ordinary
15 and contingent expenses of the Department.
16 Any person who delivers a check or other payment to the
17 Department that is returned to the Department unpaid by the
18 financial institution upon which it is drawn shall pay to the
19 Department, in addition to the amount already owed to the
20 Department, a fine of $50. If the check or other payment was
21 for a renewal or issuance fee and that person practices
22 without paying the renewal fee or issuance fee and the fine
23 due, an additional fine of $100 shall be imposed. The fines
24 imposed by this Section are in addition to any other
25 discipline provided under this Act for unlicensed practice or
26 practice on a nonrenewed license. The Department shall notify
27 the person that payment of fees and fines shall be paid to
28 the Department by certified check or money order within 30
29 calendar days of the notification. If, after the expiration
30 of 30 days from the date of the notification, the person has
31 failed to submit the necessary remittance, the Department
32 shall automatically terminate the license or certificate or
33 deny the application, without hearing. If, after termination
34 or denial, the person seeks a license or certificate, he or

1 she shall apply to the Department for restoration or issuance
2 of the license or certificate and pay all fees and fines due
3 to the Department. The Department may establish a fee for the
4 processing of an application for restoration of a license or
5 certificate to pay all expenses of processing this
6 application. The Director may waive the fines due under this
7 Section in individual cases where the Director finds that the
8 fines would be unreasonable or unnecessarily burdensome.

10 Section 50. The Detection of Deception Examiners Act is
11 amended by changing Section 26 as follows:

13 Sec. 26. Fees. The Department shall provide by rule for a
14 schedule of fees for the administration and enforcement of
15 this Act, including but not limited to original licensure,
16 renewal, and restoration. The fees shall be nonrefundable.
17 All fees collected under this Act shall be deposited into
18 the General Professions Dedicated Fund and shall be
19 appropriated to the Department for the ordinary and
20 contingent expenses of the Department in the administration
21 of this Act. The following fees shall be paid to the
22 Department for the licensing and registration functions
23 performed by the Department under this Act and shall be
24 nonrefundable:
25 (A) License as detection of deception examiner.
26 (1) The fee for application and for a license as a
27 detection of deception examiner is $25.
28 (2) In addition, applicants for any examination shall be
29 required to pay, either to the Department or to the
30 designated testing service, a fee covering the cost of
31 determining the applicant's eligibility and providing the
32 examination.

1 (3) The fee upon the renewal of a license as a detection
2 of deception examiner shall be calculated at the rate of $90
3 per year.
4 (4) The fee for the reinstatement of a license as a
5 detection of deception examiner which has expired for not
6 more than 5 years is $10 plus all lapsed renewal fees.
7 (5) The fee for the restoration of a license as a
8 detection of deception examiner which has expired for more
9 than 5 years is $10 plus all lapsed renewal fees.
10 (B) General Fees.
11 (1) The fee for the issuance of a duplicate license, for
12 the issuance of a replacement license for a license which has
13 been lost or destroyed or for the issuance of a certificate
14 with a change of name or address other than during the
15 renewal period is $10.
16 (2) The fee for a certification of a licensee's record
17 for any purpose is $10.
18 (3) The fee to have the scoring of an examination
19 administered by the Department reviewed and verified is $10
20 plus any fees charged by the applicable testing service.
21 (4) The fee for a licensee for a wall certificate shall
22 be the actual cost of producing such certificate.
23 (5) The fee for a roster of persons licensed as
24 detection of deception examiners and registered to study
25 detection of deception in this State shall be the actual cost
26 of producing such a roster.
27 This Section in regard to fees shall not apply to any
28 examiner in the exclusive employment of the United States of
29 America, the State of Illinois, any County, municipality, or
30 political subdivision in this State, any Department, Bureau

1 Section 55. The Private Detective, Private Alarm,
2 Private Security, and Locksmith Act of 1993 is amended by
3 changing Section 105 as follows:

5 Sec. 105. Issuance of license; renewal, fees.
6 (a) The Department shall, upon the applicant's
7 satisfactory completion of the requirements authorized under
8 this Act, and upon receipt of the requisite fees, issue the
9 appropriate license and wallet card showing the name and
10 business location of the licensee and the dates of issuance
11 and expiration, and containing a photograph of the licensee
12 provided to the Department that is not more than one year old
13 as of the date of application for licensure and reflects any
14 recent appearance changes.
15 (b) An applicant may upon satisfactory completion of
16 requirements authorized under this Act, and upon receipt of
17 requisite fees related to the application and testing for
18 licensure under this Act, elect to defer the issuance of the
19 applicant's initial license for a period not longer than 6
20 years. Any applicant who fails to request issuance of their
21 initial license or agency certificate and remit the fees
22 associated with that license within 6 years shall be required
23 to resubmit an applicant for licensure together with all
24 required fees.
25 (c) The expiration date, renewal period, and conditions
26 for renewal and restoration of each license, agency
27 certificate of registration, permanent employee registration
28 card, and firearm authorization card issued under this Act
29 shall be set by rule of the Department. The holder may renew
30 the license, agency certificate of registration, permanent
31 employee registration card, or firearm authorization card
32 during the month preceding its expiration by paying the
33 required fee and by meeting those conditions that the

1 Department may specify. Any license holder who notifies the
2 Department in writing, on forms prescribed by the Department,
3 may elect to place his or her license on an inactive status
4 for a period of no longer than 6 years and shall, subject to
5 the rules of the Department, be excused from payment of
6 renewal fees until he or she notifies the Department in
7 writing of a desire to resume active status. Practice while
8 on inactive status constitutes unlicensed practice. A
9 nonrenewed license that has lapsed for less than 6 years may
10 be restored upon payment of a $50 restoration fee plus all
11 lapsed renewal fees. A license that has lapsed for more than
12 6 years may be restored by paying the required fees specified
13 in this Section and by providing evidence of competence to
14 resume practice satisfactory to the Department and the Board,
15 which may include passage of the written examination. All
16 restoration fees and lapsed renewal fees shall be waived for
17 an applicant whose license lapsed while on active duty in the
18 military provided application for restoration is made within
19 one year after discharge from the service.
20 (d) The Department shall provide by rule for a schedule
21 of fees for the administration and enforcement of this Act,
22 including but not limited to original licensure, renewal, and
23 restoration. The fees shall be nonrefundable.
24 All fees collected under this Act shall be deposited into
25 the General Professions Dedicated Fund and shall be
26 appropriated to the Department for the ordinary and
27 contingent expenses of the Department in the administration
28 of this Act. The nonrefundable fees are as follows:
29 (1) The filing fee for the first examination is $25
30 plus the cost of determining an applicant's eligibility
31 and of providing the examination; subsequent examination
32 fees shall be the cost of the examination; the fee for
33 the examination shall be paid to the Department or its
34 designee.

1 (2) The initial issuance of any individual license
2 or agency certificate shall be $500.
3 (3) The initial issuance of a branch office
4 certificate is $200.
5 (4) The license issued to an applicant licensed in
6 another state or foreign country on the basis of
7 endorsement is $500.
8 (5) The triennial renewal of any individual license
9 or agency certificate is $450.
10 (6) The triennial renewal of a branch office
11 certificate is $200.
12 (7) The reinstatement of a license that has been
13 lapsed for less than 6 years is $50, plus all lapsed
14 renewal fees.
15 (8) The restoration of a lapsed license that has
16 been lapsed for more than 6 years is $100, plus all
17 lapsed renewal fees.
18 (9) The issuance of a duplicate license, agency
19 certificate of registration, permanent employee
20 registration card, certification of completion of 20-Hour
21 Basic Training, Certification of Firearm Training,
22 Firearm Authorization Card, or a certificate issued for a
23 change of name or address other than during the renewal
24 period is $25.
25 (10) The issuance of a permanent employee
26 registration card is $55; the triennial renewal of the
27 card is $45.
28 (11) The issuance of a firearm authorization card
29 is $55.
30 (12) The triennial renewal for a Firearm
31 Authorization Card is $45; reissuance of a Firearm
32 Authorization Card to an agency that has changed its name
33 is $10.00.
34 (13) For processing a fingerprint card by the State

1 Police the fee is the cost of processing, which shall be
2 made payable to the State Police Services Fund and shall
3 be remitted to the State Police for deposit into the
4 Fund.
5 (14) For review and verification of the scoring of
6 an examination administered by the Department, $20 plus
7 any actual fees charged by the testing service.
8 (15) For a roster of licensees or registrants the
9 fee shall be the cost of producing a roster.
10 (16) The fee for issuance of certification of a
11 license record shall be $20.
12 (17) The fee for issuance and renewal of an armed
13 proprietary security force registration is $20.
14 (18) The fee for reinstatement of a license from
15 inactive status that has lapsed for a period less than 6
16 years shall be the same as the current renewal fee.

18 Section 99. Effective date. This Act takes effect
