1 AMENDMENT TO SENATE BILL 1295
2 AMENDMENT NO. . Amend Senate Bill 1295 by replacing
3 everything after the enacting clause with the following:
4 "Section 5. The University of Illinois Act is amended by
5 adding Section 20 as follows:
6 (110 ILCS 305/20 new)
7 Sec. 20. Release of educational records; consent. For
8 each semester, the University shall include on an
9 undergraduate student's tuition bill or other statement of
10 tuition charges or as a separate attachment to the bill or
11 charge a consent form that permits the student to certify in
12 writing that the student consents to having a copy of his or
13 her educational records, as defined under Section 1232g of
14 the General Education Provisions Act (20 U.S.C. 1232g), sent
15 to a person designated by the student, at an address
16 designated by the student, or otherwise made available to the
17 designated person. The consent form shall comply with the
18 requirements in Section 1232g(b)(2)(A) of the General
19 Education Provisions Act (20 U.S.C. 1232g(b)(2)(A)). The
20 student shall have the option of refusing consent by not
21 completing the consent form. If the student consents, the
22 University shall send or otherwise make available a copy of

1 the educational records to the person designated by the
2 student upon that person's request.
3 Section 10. The Southern Illinois University Management
4 Act is amended by adding Section 10 as follows:
5 (110 ILCS 520/10 new)
6 Sec. 10. Release of educational records; consent. For
7 each semester, the University shall include on an
8 undergraduate student's tuition bill or other statement of
9 tuition charges or as a separate attachment to the bill or
10 charge a consent form that permits the student to certify in
11 writing that the student consents to having a copy of his or
12 her educational records, as defined under Section 1232g of
13 the General Education Provisions Act (20 U.S.C. 1232g), sent
14 to a person designated by the student, at an address
15 designated by the student, or otherwise made available to the
16 designated person. The consent form shall comply with the
17 requirements in Section 1232g(b)(2)(A) of the General
18 Education Provisions Act (20 U.S.C. 1232g(b)(2)(A)). The
19 student shall have the option of refusing consent by not
20 completing the consent form. If the student consents, the
21 University shall send or otherwise make available a copy of
22 the educational records to the person designated by the
23 student upon that person's request.
24 Section 15. The Chicago State University Law is amended
25 by adding Section 5-115 as follows:
26 (110 ILCS 660/5-115 new)
27 Sec. 5-115. Release of educational records; consent.
28 For each semester, the University shall include on an
29 undergraduate student's tuition bill or other statement of
30 tuition charges or as a separate attachment to the bill or

1 charge a consent form that permits the student to certify in
2 writing that the student consents to having a copy of his or
3 her educational records, as defined under Section 1232g of
4 the General Education Provisions Act (20 U.S.C. 1232g), sent

7 designated person. The consent form shall comply with the
8 requirements in Section 1232g(b)(2)(A) of the General
9 Education Provisions Act (20 U.S.C. 1232g(b)(2)(A)). The

13 educational records to the person designated by the student
14 upon that person's request.
15 Section 20. The Eastern Illinois University Law is
16 amended by adding Section 10-115 as follows:

5 amended by adding Section 15-115 as follows:

12 charge a consent form that permits the student to certify in
13 writing that the student consents to having a copy of his or
14 her educational records, as defined under Section 1232g of
15 the General Education Provisions Act (20 U.S.C. 1232g), sent
16 to a person designated by the student, at an address
17 designated by the student, or otherwise made available to the
18 designated person. The consent form shall comply with the
19 requirements in Section 1232g(b)(2)(A) of the General
20 Education Provisions Act (20 U.S.C. 1232g(b)(2)(A)). The
21 student shall have the option of refusing consent by not

26 Section 45. The Western Illinois University Law is
27 amended by adding Section 35-120 as follows:

30 For each semester, the University shall include on an

8 designated by the student, or otherwise made available to the

17 Section 99. Effective date. This Act takes effect upon
18 becoming law.".
