AN ACT concerning health.
|
Be it enacted by the People of the State of Illinois,
|
represented in the General Assembly:
|
Section 5. The Mental Health and Developmental |
Disabilities Code is amended by changing Sections 3-201 and |
4-202 as follows:
|

Sec. 3-201.
The Department shall prescribe all forms |
necessary for
proceedings under this Chapter, and all forms |
used in such proceedings shall
comply substantially with the |
forms so prescribed. The Department shall
publish all forms in |
electronic format and post the forms to its website print and |
furnish an initial supply of such forms to the clerks of the |
circuit courts.
