1 | | AN ACT concerning regulation.

2 | | Be it enacted by the People of the State of Illinois, |
3 | | represented in the General Assembly:
|
4 | | Section 5. The Illinois Dental Practice Act is amended by |
5 | | changing Section 8.05 as follows:
|
6 | | (225 ILCS 25/8.05)

7 | | (Section scheduled to be repealed on January 1, 2026)

8 | | Sec. 8.05. Social Security Number on license application. |
9 | | In addition
to any other information required to be contained |
10 | | in the the application, every
application for an original |
11 | | license under this Act shall
include the applicant's Social |
12 | | Security Number, which shall be retained in the agency's |
13 | | records pertaining to the license. As soon as practical, the |
14 | | Department shall assign a customer's identification number to |
15 | | each applicant for a license. |
16 | | Every application for a renewal or restored license shall |
17 | | require the applicant's customer identification number.
