| 2 | | AMENDMENT NO. ______. Amend House Bill 2723 on page 1, line |
| 3 | | 5, by replacing "21-101" with "21-101, 21-102,"; and

| 4 | | on page 1, line 18, after the period, by inserting "A person |
| 5 | | who has been convicted of any offense for which a person is |
| 6 | | required to register under the Sex Offender Registration Act, |
| 7 | | Murderer and Violent Offender Against Youth Registration Act, |
| 8 | | or Arsonist Registration Act in this State or any other state |
| 9 | | who has not been pardoned shall not be permitted to file a |
| 10 | | petition for a name change in the courts of Illinois during the |
| 11 | | period he or she is required to register."; and

| 12 | | on page 3, by inserting immediately below line 21 the |
| 13 | | following:
|
| 14 | | "(735 ILCS 5/21-102) (from Ch. 110, par. 21-102)

| 1 | | (a) The petition shall set forth the name then held,
the |
| 2 | | name sought to be assumed, the residence of the petitioner, the |
| 3 | | length
of time the petitioner has resided in this State, and |
| 4 | | the state or country
of the petitioner's nativity or supposed |
| 5 | | nativity. The petition shall be
signed by the person |
| 6 | | petitioning or, in case of minors, by the parent or
guardian |
| 7 | | having the legal custody of the minor. The petition shall be
|
| 8 | | verified by the affidavit of some credible person. |
| 9 | | (b) Any petition form that a person may file as a petition |
| 10 | | under this Article by filling in the blanks in the form with |
| 11 | | his or her information shall contain the following: "WARNING: A |
| 12 | | person who is required to register under the Sex Offender |
| 13 | | Registration Act, Murderer and Violent Offender Against Youth |
| 14 | | Registration Act, or Arsonist Registration Act in this State or |
| 15 | | similar law in any other state who has not been pardoned |
| 16 | | commits a felony offense under those respective Acts by seeking |
| 17 | | to change his or her name during his or her registration |
| 18 | | period."
