1 | SENATE RESOLUTION
| ||||||
2 | WHEREAS, The members of the Illinois Senate are saddened to | ||||||
3 | learn of the death of Lisa Ann LaForge, who passed away on June | ||||||
4 | 29, 2017; and
| ||||||
5 | WHEREAS, Lisa LaForge spent her career as a social worker, | ||||||
6 | and she was committed to helping the clients of the Family | ||||||
7 | Service Association of Greater Elgin, where she was the | ||||||
8 | Executive Director; she worked tirelessly with the Board, the | ||||||
9 | leadership team of the association, and staff to improve the | ||||||
10 | lives of many; and
| ||||||
11 | WHEREAS, One of Lisa LaForge's crowning achievements was | ||||||
12 | the instrumental role she played in starting the Screening, | ||||||
13 | Assessment, and Support Services Program in the State of | ||||||
14 | Illinois; she was the leader that brought the program to Family | ||||||
15 | Service Association; she worked together with Illinois | ||||||
16 | officials for many years to bring together a crisis system that | ||||||
17 | was, and still is, responsive to youth experiencing significant | ||||||
18 | mental health distress; and
| ||||||
19 | WHEREAS, Lisa LaForge also played a substantial role in the | ||||||
20 | Mental Health Juvenile Justice Program; she worked together | ||||||
21 | with local courts, the juvenile detention center, and probation | ||||||
22 | departments to create a work flow that allowed for youth to |

1 | receive community based services prior to their release from | ||||||
2 | the detention center; she was a pioneer, and she won the Elgin | ||||||
3 | Image Award for her outstanding work in advocacy while also | ||||||
4 | running a highly reputable, financially solvent organization | ||||||
5 | that responds to community needs; and
| ||||||
6 | WHEREAS, Lisa LaForge was a good friend, mentor and | ||||||
7 | colleague to many; she was a dedicated daughter, sister, and | ||||||
8 | aunt; she was fun and funny, regaling all with a wicked sense | ||||||
9 | of humor and an infectious laugh; and
| ||||||
10 | WHEREAS, Lisa LaForge is survived by her parents, Peter and | ||||||
11 | Peggy LaForge; her brothers, Philip (Patricia) and Steven | ||||||
12 | (Julie); her nieces and nephews, Peter, Claire, Jonathan, and | ||||||
13 | Lorisa; and many cousins; therefore, be it
| ||||||
14 | RESOLVED, BY THE SENATE OF THE ONE HUNDREDTH GENERAL | ||||||
15 | ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of | ||||||
16 | Lisa Ann LaForge and extend our sincere condolences to her | ||||||
17 | family, friends, and all who knew and loved her; and be it | ||||||
18 | further

19 | RESOLVED, That a suitable copy of this resolution be | ||||||
20 | presented to the family of Lisa LaForge as an expression of our | ||||||
21 | deepest sympathy.
