4 information from the Department of Public Health's website on

6 on page 1, line 8, by replacing "with certain patients" with

7 "and the opioid alternative program with patients"; and

8 on page 1, by replacing lines 15 through 18 with "that is a

9 Schedule II controlled substance, provide information from the

10 Department of Public Health's website on the"; and

11 on page 1, by replacing lines 20 and 21 with the following:

12 "the opioid and the alternative program managed by the

13 Department of Public Health to the patient."; and

14 on page 1, line 22, by replacing "engages in a discussion" with

9 under the Veterinary Medicine and Surgery Practice Act of

10 2004.".
