| 5 | | to a child under the school-based dental program, the provider |
| 6 | | must enter into a written agreement with a dentist located |
| 7 | | within the child's school district. The written agreement must |
| 8 | | set out the specific arrangement under which follow-up care |
| 9 | | will be provided to the child.".
