| 2 | | WHEREAS, The members of the Illinois Senate are saddened to |
| 3 | | learn of the death of Dr. Thomas E. "Tom" Theobald OD of |
| 4 | | Jacksonville, Florida, formerly of Bloomington, who passed |
| 5 | | away on February 2, 2019; and
|
| 6 | | WHEREAS, Tom Theobald was born in Springfield to Paul and |
| 7 | | Marguerite (Phillips) Theobald on October 15, 1940; he |
| 8 | | graduated from Tri-City High School; he then earned his |
| 9 | | Bachelor of Science from Culver-Stockton College, where he was |
| 10 | | also a member of Sigma Phi Epsilon; he earned his Doctor of |
| 11 | | Optometry degree from Southern College of Optometry, where he |
| 12 | | was a member of Omega Delta; he married Joyce McMaster in |
| 13 | | Memphis, Tennessee on July 6, 1963; she preceded him in death; |
| 14 | | he later married Lynn Morford in Buffalo Hart on March 22, |
| 15 | | 2003; and
|
| 16 | | WHEREAS, Tom Theobald was a Captain in the Army Medical |
| 17 | | Service Corps and served as an optometry officer at Valley |
| 18 | | Forge General Hospital in Pennsylvania during the Vietnam era; |
| 19 | | he had an optometry practice in Normal and worked as an adjunct |
| 20 | | professor at Illinois State University; and
|
| 21 | | WHEREAS, Tom Theobald was active in many civic |
| 22 | | organizations; he served as Chairman Emeritus of Citizens |
|

| 1 | | Savings Bank (now Busey Bank) and as Vice Chairman of the |
| 2 | | Bloomington-Normal Sanitary District; he was a fellow of the |
| 3 | | American Academy of Optometry and held several offices in local |
| 4 | | and state optometric associations and the American Optometric |
| 5 | | Association; as President of the Normal Chamber of Commerce, he |
| 6 | | worked to create the McLean County Association of Commerce and |
| 7 | | Industry; he was a former member of the Normal Rotary Club and |
| 8 | | a former board member of the Bloomington Country Club; and
|
| 9 | | WHEREAS, Tom Theobald enjoyed creating computer |
| 10 | | applications with his friend, Dr. Gerry McKean; he earned his |
| 11 | | commercial pilot's license and enjoyed golfing, scuba diving, |
| 12 | | traveling, watchmaking, and, most of all, his family; and
|
| 13 | | WHEREAS, Tom Theobald was preceded in death by his sister, |
| 14 | | Linda (Paul) Smith; and
|
| 15 | | WHEREAS, Tom Theobald is survived by his wife, Lynn; his |
| 16 | | daughters, Ann (Brent) Hollenbeck and Jane (Marc) Stein; his |
| 17 | | brother, Dale (Linda) Theobald; his grandchildren, Kate |
| 18 | | Hollenbeck, Olivia Hollenbeck, Ellee Stein, and Nathan Stein; |
| 19 | | his sister-in-law, Jane (Clay) Wilson; his brother-in-law, |
| 20 | | Scott (Susan) Morford; and several cousins, nieces, and |
| 21 | | nephews; therefore, be it
