| 1 | | AN ACT concerning health.
|
| 2 | | Be it enacted by the People of the State of Illinois,
|
| 3 | | represented in the General Assembly:
|
| 4 | | Section 5. The Mental Health and Developmental |
| 5 | | Disabilities Code is amended by adding Section 3-911 as |
| 6 | | follows: |
| 7 | | (405 ILCS 5/3-911 new) |
| 8 | | Sec. 3-911. Aftercare plan. |
| 9 | | (a) The Department of Human Services when a recipient of |
| 10 | | services under this Code, whether admitted on a voluntary or |
| 11 | | involuntary basis, is being discharged from an inpatient |
| 12 | | facility, shall provide the recipient and the recipient's |
| 13 | | conservator, guardian, or other legally authorized |
| 14 | | representative a written aftercare plan prior to the |
| 15 | | recipient's discharge from the facility.
The written aftercare |
| 16 | | plan shall include, to the extent known, all of the following |
| 17 | | components: |
| 18 | | (1) the nature of the illness and followup required; |
| 19 | | (2) medications including side effects and dosage |
| 20 | | schedules; |
| 21 | | (3) if the recipient was given an informed consent |
| 22 | | form with his or her medications, the form shall satisfy |
| 23 | | the requirement for information on side effects of the |
