| 3 | | on page 5, immediately below line 5, by inserting the |
| 4 | | following: |
| 5 | | "If you have been selected off of the Prioritization of |
| 6 | | Urgency of Need for Services (PUNS) waiting list to receive |
| 7 | | State-funded developmental disabilities services, please |
| 8 | | contact your service coordination agency for advice before you |
| 9 | | take any action in response to this notice."; and

| 10 | | on page 6, immediately below line 20, by inserting the |
| 11 | | following:
|
| 12 | | "(g) This Section does not apply to any student who is no |
| 13 | | longer a resident of the school district that was responsible |
| 14 | | for the student's individualized education program at the time |
| 15 | | the student reached the student's 22nd birthday.".
