| 1 | | Department must keep a record of the social
security number of |
| 2 | | each applicant. The Department shall notify the applicant
that |
| 3 | | his or her social security number is kept on file with the |
| 4 | | Department.

| 5 | | (c) A licensee's social security number shall not appear |
| 6 | | on the face of his
or her license.

| 7 | | (d) The Department shall offer to each applicant for a |
| 8 | | hunting license issued under the Wildlife Code the opportunity |
| 9 | | to have his or her name included in the First Person Consent |
| 10 | | organ and tissue donor registry maintained by the Secretary of |
| 11 | | State under Section 6-117 of the Illinois Vehicle Code. The |
| 12 | | Department must advise the applicant that he or she is under no |
| 13 | | compulsion to have his or her name included in the registry. An |
| 14 | | individual who agrees to have his or her name included in the |
| 15 | | First Person Consent organ and tissue donor registry has given |
| 16 | | full legal consent to the donation of any of his or her organs |
| 17 | | or tissue upon his or her death. A brochure explaining this |
| 18 | | method of executing an anatomical gift must be given to each |
| 19 | | applicant for a hunting license issued under the Wildlife |
| 20 | | Code. The brochure must advise the applicant (i) that he or she |
| 21 | | is under no compulsion to have his or her name included in this |
| 22 | | registry; and (ii) that he or she may wish to consult with |
| 23 | | family, friends, or clergy before doing so. The Department is |
| 24 | | authorized to collect and compile the names of individuals |
| 25 | | that agree to be included in the First Person Consent organ and |
| 26 | | tissue donor registry and provide the information to the |
