| 1 | | AN ACT concerning transportation.
|
| 2 | | Be it enacted by the People of the State of Illinois,
|
| 3 | | represented in the General Assembly:
|
| 4 | | Section 5. The Illinois Vehicle Code is amended by |
| 5 | | changing Section 3-643 as follows:
|
| 6 | | (625 ILCS 5/3-643)
|
| 7 | | Sec. 3-643. Mammogram license plates.
|
| 8 | | (a) The Secretary, upon receipt of an application made in |
| 9 | | the form
prescribed by the Secretary, may issue special |
| 10 | | registration plates designated
as Mammogram license plates. |
| 11 | | The special plates issued under this Section
shall be affixed |
| 12 | | only to passenger vehicles of the first division and motor
|
| 13 | | vehicles of the second division weighing not more than 8,000 |
| 14 | | pounds. Plates
issued under this Section shall expire |
| 15 | | according to the multi-year procedure
established by Section |
| 16 | | 3-414.1 of this Code.
|
| 17 | | (b) The design and color of the plates is wholly within the |
| 18 | | discretion of
the Secretary, except that the phrase |
| 19 | | "Mammograms Save Lives" following phrases shall be on the |
| 20 | | plates: (i)
"Mammograms Save Lives" and (ii) "The Susan G. |
| 21 | | Komen Foundation". The
Secretary may allow the plates to be |
| 22 | | issued as vanity plates or personalized
under Section 3-405.1 |
| 23 | | of the Code. The Secretary shall prescribe stickers or
decals |

| 1 | | as provided under Section 3-412 of this Code.
|
| 2 | | (c) An applicant for the special plate shall be charged a |
| 3 | | $25 fee for
original issuance in addition to the appropriate |
| 4 | | registration fee. Of this
fee, $10 shall be deposited into the |
| 5 | | Mammogram Fund and $15 shall be
deposited into the Secretary |
| 6 | | of State Special License Plate Fund, to be used by
the |
| 7 | | Secretary to help defray the administrative processing costs.
|
| 8 | | For each registration renewal period, a $25 fee, in |
| 9 | | addition to the
appropriate registration fee, shall be |
| 10 | | charged. Of this fee, $23 shall be
deposited into the |
| 11 | | Mammogram Fund and $2 shall be deposited into the
Secretary of |
| 12 | | State Special License Plate Fund.

| 13 | | (d) The Mammogram Fund is created as a special fund in the |
| 14 | | State
treasury. All money in the Mammogram Fund shall be paid, |
| 15 | | subject to
appropriation by the General Assembly
and |
| 16 | | distribution by the Secretary, to the Illinois Breast and |
| 17 | | Cervical Cancer Program for patient navigation services |
| 18 | | specifically for populations with the highest rates of breast |
| 19 | | cancer mortality in the State as grants to the Susan G. Komen |
| 20 | | Foundation for
breast cancer research, education, screening, |
| 21 | | and treatment.
