| 2 | | AMENDMENT NO. ______. Amend Senate Bill 1592 on page 3, by |
| 3 | | replacing lines 2 through 5 with the following:

| 4 | | "(e-5) An insurer may not deny or refuse to provide |
| 5 | | otherwise covered services under a group or individual policy |
| 6 | | of accident and health insurance or a managed care plan solely |
| 7 | | because of the location wherein the clinically appropriate |
| 8 | | services are provided."; and

| 9 | | on page 11, by replacing lines 11 through 14 with the |
| 10 | | following: |
| 11 | | "(j) An insurer may not deny or refuse to provide
|

|
