Amends the Medical Assistance Article of the Illinois Public Aid Code. Provides that, subject to federal approval, the Department of
Healthcare and Family Services, in cooperation with the
Department of Human Services' Division of Developmental Disabilities, shall establish a Bridge to Community Supports
for Young Adults with Developmental Disabilities Waiver
Program (waiver program). Provides that the waiver program shall serve young adults with
developmental disabilities who are at least 18 years of age but
under 27 years of age from the time they graduate from high
school or are no longer eligible for special education services
until they are removed from the Prioritization for Urgency of
Need for Services database or until the day before their
27th birthday, whichever occurs first. Provides that the waiver program shall provide an array of
self-directed service choices, based on a person-centered plan
developed with the eligible young adult, at a maximum of $15,000 per
individual per year. Requires the Department of Healthcare and Family Services to apply to the federal Centers for Medicare and Medicaid
Services for a research and demonstration waiver under the Social Security Act to establish the waiver program. Provides that implementation of the waiver program is subject to federal approval. Effective immediately.

2 | | Be it enacted by the People of the State of Illinois,

3 | | represented in the General Assembly:

4 | | Section 5. The Illinois Public Aid Code is amended by |
5 | | adding Section 5-41 as follows: |

7 | | Sec. 5-41. Bridge to Community Supports for Young Adults |
8 | | with Developmental Disabilities Waiver Program. |
9 | | (a) The purpose of the waiver program created under this |
10 | | Section is to provide services to individuals with |
11 | | developmental disabilities during the period after they |
12 | | graduate from high school until they qualify for adult |
13 | | services. |
14 | | (b) Subject to federal approval, the Department of |
15 | | Healthcare and Family Services, in cooperation with the |
16 | | Department of Human Services' Division of Developmental |
17 | | Disabilities, shall establish a Bridge to Community Supports |
