AMENDMENT TO SENATE BILL 3910




2    AMENDMENT NO. ______. Amend Senate Bill 3910 on page 2, 
3line 3, after "plan", by inserting "; for the purpose of this 
4requirement, the Department of Healthcare and Family Services 
5is the regulatory entity that holds authority over plans that 
6the Department of Healthcare and Family Services has 
7contracted with to provide services under the medical 
8assistance program"; and


 
9on page 3, by replacing line 16 through line 23 with the 
10following:
11        "(6)   the cardholder's name or a space to permit the 
12    cardholder to print his or her name, if the cardholder 
13    pays a periodic charge for use of the card;
14        (7)   a processor control number, if required for claims 
15    adjudication; and

16        (8)   a statement that the plan is not insurance."; and
