| 4 | | passed away on January 4, 2021; and

| 5 | | WHEREAS, Jack Millen was born to Arlo and Virginia Millen |
| 6 | | in Moline on February 21, 1935; he served in the United States |
| 7 | | Army; he married Sharron L. Frazell in Peoria on January 21, |
| 8 | | 1961; and
|
| 9 | | WHEREAS, Jack Millen was a member of and served as an elder |
| 10 | | and as financial secretary at Immanuel Lutheran Church; he |
| 11 | | served as grounds maintenance for the Immanuel Lutheran Church |
| 12 | | Cemetery; and
|
| 13 | | WHEREAS, Jack Millen was a Chicago Cubs fan and loved |
| 14 | | bowling and golf; and
|
| 15 | | WHEREAS, Jack Millen was preceded in death by his parents; |
| 16 | | his sister, Vanetta Nelson; and his brother, Erwin Millen; and
|
| 17 | | WHEREAS, Jack Millen is survived by his wife; his |
| 18 | | children, Karen (John) Cain, Kevin Millen, and Stacy (Tom) |
| 19 | | Biscontine; his grandchildren, Nicole (Paul) Krauss, Brian |
| 20 | | (Meredith) Cain, and Sydney Biscontine; and his siblings, |
