| | | Amends the Vital Records Act. Provides that an individual's status as a person under guardianship with the Office of State Guardian may be verified with a copy of the court order placing the individual under the guardianship of the Office. Provides that applicable fees for a new birth certificate and search for a birth record or certified copy of a birth record shall be waived for all requests made by the Office for an individual under guardianship of the Office. Provides that the State Registrar of Vital Records shall establish standards and procedures for waiver of the applicable fees. Provides that an individual under guardianship of the Office shall be provided no more than 4 birth records annually. |

| 2 | | Be it enacted by the People of the State of Illinois, |
| 3 | | represented in the General Assembly:

| 4 | | Section 5. The Vital Records Act is amended by adding |
| 5 | | Section 25.2 as follows:
|
| 6 | | (410 ILCS 535/25.2 new) |
| 7 | | Sec. 25.2. Office of State Guardian birth record request. |
| 8 | | (a) For purposes of this Section, an individual's status |
| 9 | | as a person under guardianship with the Office of State |
| 10 | | Guardian may be verified with a copy of the court order placing |
| 11 | | the individual under the guardianship of the Office of State |
| 12 | | Guardian. |
| 13 | | (b) The applicable fees under Section 17 for a new |
| 14 | | certificate of birth and under Section 25 for a search for a |
| 15 | | birth record or certified copy of a birth record shall be |
| 16 | | waived for all requests made by the Office of State Guardian |
| 17 | | for an individual under guardianship of the Office of State |
| 18 | | Guardian, whose status is verified under subsection (a) of |
| 19 | | this Section. |
| 20 | | (c) The State Registrar of Vital Records shall establish |
| 21 | | standards and procedures consistent with this Section for |
| 22 | | waiver of the applicable fees. |
