| 1 | | AMENDMENT TO SENATE BILL 3208

| 2 | | AMENDMENT NO. ______. Amend Senate Bill 3208 on page 4, by |
| 3 | | replacing lines 19 and 20 with "the pay stubs to the employee |
| 4 | | within 21 calendar days of the employee's request. An employer |
| 5 | | is"; and

| 6 | | by replacing line 26 on page 4 and line 1 on page 5 with "copy |
| 7 | | of the pay stubs to the former employee within 21 calendar days |
| 8 | | of the former employee's"; and

| 9 | | on page 5, immediately below line 22, by inserting the |
| 10 | | following: |
| 11 | | "(4) A request made by an employee or former employee |
| 12 | | under this Section shall be made to a person responsible |
| 13 | | for maintaining the employer's payroll, including the |
| 14 | | employer's human resources department or payroll |
| 15 | | department, the employee's supervisor or department |
| 16 | | manager, or an individual designated in the employer's |
