| 3 | | by replacing everything after the enacting clause with the |

| 5 | | "Section 5. The Assisted Living and Shared Housing Act is |
| 6 | | amended by adding Section 92 as follows:

| 7 | | (210 ILCS 9/92 new) |
| 8 | | Sec. 92. Incident and accident reporting. An establishment |
| 9 | | must report to the Department any incident or accident that |
| 10 | | results in significant physical harm or injury to a resident |
| 11 | | or any situation where a resident requires outside emergent |
| 12 | | medical treatment as a direct result of an incident or |
| 13 | | accident. A change in a resident's condition that is due to |
| 14 | | health or medical decline is not a reportable incident or |
| 15 | | accident.". |
