| 4 | | on page 7, by replacing line 19 with the following: |

| 6 | | (8) the cost of the Department's end of life care for |
| 7 | | committed persons who died and were not in hospice or |
| 8 | | palliative care programs, including the following: |
| 9 | | (A) the annual costs associated with end of life |
| 10 | | care for committed persons who died and were not in |
| 11 | | hospice or palliative care programs; |
| 12 | | (B) the sources of funding for end of life care for |
| 13 | | committed persons who died and were not in hospice or |
| 14 | | palliative care programs; and |
| 15 | | (C) the annual costs associated with end of life |
| 16 | | care that was not palliative or hospice care at each |
| 17 | | Department institution and facility.". |
