| 2 | | license-pending general dentist if all of the following |
| 3 | | criteria are met: |
| 4 | | (1) the applicant has completed and passed the |
| 5 | | Department-approved licensure examination and presents to |
| 6 | | the employer an official written notification indicating |
| 7 | | successful passage of the licensure examination; |
| 8 | | (2) the applicant has completed and submitted to the |
| 9 | | Department an application for a general dentist license |
| 10 | | under this Act; and |
| 11 | | (3) the applicant has submitted the required licensure |
| 12 | | fee. |
| 13 | | (b) An applicant's authorization to practice under this |
| 14 | | Section shall terminate upon any of the following: |
| 15 | | (1) receipt of a general dentist license from the |
| 16 | | Department; |
| 17 | | (2) notification from the Department that the |
| 18 | | applicant's application for licensure has been denied; |
| 19 | | (3) a request by the Department that the applicant |
| 20 | | terminate practicing as a license-pending general dentist |
| 21 | | until an official decision is made by the Department to |
| 22 | | grant or deny a general dentist license to the applicant; |
| 23 | | or |
| 24 | | (4) 6 months elapsing since the official date of the |
| 25 | | applicant's passage of the licensure examination, as |
| 26 | | specified on the formal written notification provided to |

| 1 | | the applicant upon passage of the examination. The 6-month |
| 2 | | license-pending period may be extended by the Department |
| 3 | | by rule. |

| 5 | | Sec. 13.4. Practice of license-pending dental hygienists. |
| 6 | | (a) An applicant for licensure as a dental hygienist under |
| 7 | | this Act may obtain employment as a license-pending dental |
| 8 | | hygienist and practice under the delegation of a licensed |
| 9 | | general dentist. An individual may be employed as a |
| 10 | | license-pending dental hygienist if all of the following |
| 11 | | criteria are met: |
| 12 | | (1) the applicant has completed and passed the |
| 13 | | Department-approved licensure examination and presents to |
| 14 | | the employer an official written notification indicating |
| 15 | | successful passage of the licensure examination; |
| 16 | | (2) the applicant has completed and submitted to the |
