| 1 | | of care includes the following components: |
| 2 | | (1) Comprehensive case management. Each PCIN |
| 3 | | organization shall provide comprehensive case management |
| 4 | | services. Comprehensive case management shall be driven by |
| 5 | | a holistic household level needs assessment. Case |
| 6 | | management is family based and trauma informed and is |
| 7 | | geared toward self-sufficiency and community integration. |
| 8 | | (2) Core integrated services. Each PCIN organization |
| 9 | | must provide the following core integrated services: |
| 10 | | (A) trauma-informed family-based case management; |
| 11 | | (B) access to health care coverage navigation; |
| 12 | | (C) access to counseling and behavioral health |
| 13 | | services (including peer support models); |
| 14 | | (D) career development and job readiness; and |
| 15 | | (E) legal assistance. |
| 16 | | If the PCIN organization does not provide all services |
| 17 | | outlined in subparagraphs (A) through (E), then it shall |
| 18 | | establish formal agreements with partner organizations |
| 19 | | that are able to provide such services using a co-location |
| 20 | | model whenever possible. All services are provided based |
| 21 | | on individual client eligibility. |
| 22 | | (3) Co-location of services. To maximize resources and |
| 23 | | provide holistic care, the PCIN organization shall |
| 24 | | leverage publicly and privately funded services to address |
| 25 | | the needs of the client population and to be provided |
| 26 | | onsite whenever possible. Priority services for |

| 1 | | co-location are determined by the client population's |
| 2 | | current and emerging needs. |
| 3 | | (4) Community education. The PCIN shall coordinate |
| 4 | | workshops, training, and technical assistance |
| 5 | | opportunities to build the capacity of LEP community |
| 6 | | residents and the community providers or institutions that |
| 7 | | work with them. Community education shall focus on areas |
| 8 | | that increase community integration and improve engagement |
| 9 | | of LEP populations. |
| 10 | | (5) Community alliance. The PCIN shall establish and |
| 11 | | facilitate a community planning group which brings |
| 12 | | together local actors, including local government, |
| 13 | | schools, colleges, community-business organizations, |
| 14 | | health facilities, and others, to respond to emergent |
| 15 | | issues relevant to LEP residents in the community, and |
| 16 | | develop services, programs, initiatives, and policies that |
| 17 | | improve supports and community integration for these |
| 18 | | individuals and families. |
| 19 | | (c) PCIN capacity building. As deemed necessary, the |
| 20 | | Department may engage with technical assistance providers to |
| 21 | | build the capacity of the PCIN organizations and the network |
| 22 | | as a whole. |
| 23 | | (d) PCIN certification. The Department shall establish a |
| 24 | | PCIN formal certification process that outlines the standards |
| 25 | | for recognition within 2 years of the PCIN program's |
