AMENDMENT TO HOUSE BILL 3823

2    AMENDMENT NO. ______. Amend House Bill 3823, AS AMENDED,
3by replacing everything after the enacting clause with the
4following:
 
5    "Section 5. The Department of Human Services Act is
6amended by adding Section 10-68 as follows:
 
7    (20 ILCS 1305/10-68 new)
8    Sec. 10-68. Pathways for Community Integration Network.
9    (a) The Pathways for Community Integration Network (PCIN)
10program is established within the Department of Human
11Services. The program shall create a network of multilingual
12community integration providers providing access to an array
13of services to remove barriers to opportunity and to foster
14pathways to safety, well-being, and economic security for the
15State's Limited English Proficient (LEP) and related
16populations.

1    (b) PCIN model. Each PCIN organization must adhere to the
2PCIN model and, with partner organizations, deliver the full
3model of care. The PCIN model of care includes the following
4components:
5        (1) Comprehensive case management. Comprehensive case
6    management shall be driven by a holistic household level
7    needs assessment. Case management is family based and
8    trauma informed and is geared toward self-sufficiency and
9    community integration.
10        (2) Core integrated services:
11            (A) trauma-informed family-based case management;
12            (B) access to health care coverage navigation;
13            (C) access to counseling and behavioral health
14        services (including peer support models);
15            (D) career development and job readiness; and
16            (E) legal assistance.
17        If the PCIN organization does not provide all services
18    outlined in subparagraphs (A) through (E), then it shall
19    refer clients to partner organizations with which the PCIN
20    has established relationships or that are part of the
21    Community Alliance, to provide such services. PCINs are
22    encouraged to use a co-location model, which may include
23    partner or Community Alliance organizations offering
24    services at their locations, when practicable. All
25    services should be provided based on the individual
26    client's needs.

1        (3) Co-location of services. To maximize resources and
2    provide holistic care, the PCIN organization shall
3    leverage publicly and privately funded services to address
4    the needs of the client population that are encouraged to
5    be provided onsite when practicable.
6        (4) Community education. The PCIN shall coordinate
7    workshops, training, and technical assistance
8    opportunities to build the capacity of LEP and related
9    populations in the community and the community providers
10    or institutions that work with them. Community education
11    shall focus on areas that increase community integration
12    and improve engagement.
13        (5) Community Alliance. The PCIN shall establish and
14    facilitate a community planning group which may bring
15    together local actors, including local government,
16    schools, colleges, community-business organizations,
17    health facilities, and others, to respond to emergent
18    issues relevant to LEP and related populations in the
19    community, and develop services, programs, initiatives,
20    and policies that improve supports and community
21    integration for these individuals and families.
22    (c) PCIN capacity building. As deemed necessary, the
23Department may engage with technical assistance providers to
24build the capacity of the PCIN organizations and the network
25as a whole.
26    (d) PCIN certification. The Department shall establish a

1PCIN formal certification process that outlines the standards
2for recognition within 2 years of the PCIN program's
3implementation. A community organization must demonstrate
4capacity to deliver all program components outlined in this
5Section to be awarded certification and be formally recognized
6as a PCIN provider.
7    (e) Rules. The Department may adopt any rules necessary to
8implement the PCIN program.".
