| 2 | | AMENDMENT NO. ______. Amend House Bill 5001 on page 4, |
| 3 | | immediately below line 25, by inserting the following: |
| 4 | | "(a-15) Notwithstanding any age requirement set forth in |
| 5 | | this Section, coverage shall be consistent with evidence-based |
| 6 | | clinical guidelines, including, but not limited to, guidelines |
| 7 | | established by the National Comprehensive Cancer Network, and |
| 8 | | shall be provided in accordance with the determination of a |
| 9 | | health care provider, including coverage for individuals under |
| 10 | | 35 years of age when appropriate. Nothing in this Section |
| 11 | | shall be construed to limit coverage otherwise required under |
| 12 | | this Section based on age.". |
