| 3 | | on page 5, by replacing lines 8 through 17 with the following: |

| 5 | | petitioner shall be required to accompany his or"; and

| 6 | | on page 5, immediately below line 19 by inserting the |
| 7 | | following: |
| 8 | | "(a-4) The adoptive parent or legal guardian of an adopted |
| 9 | | or surrendered person under the age of 21 may also petition the |
| 10 | | court for the appointment of a confidential intermediary for |
| 11 | | purposes of obtaining identifying information or arranging |
| 12 | | contact with a mutually consenting adoptive parent or legal |
| 13 | | guardian of a birth sibling of the petitioner's adopted or |
| 14 | | surrendered child under the age of 21.". |
