| 7 | | long-term disability income insurance in this State that |
| 8 | | limits the duration of coverage for mental health and |
| 9 | | substance use disorders shall be offered without a disclosure, |
| 10 | | provided at the time of application, that includes a |
| 11 | | notification stating that:"; and

| 12 | | on page 2, by replacing line 10 with "coverage.". |
