| | | Amends the Administration Article of the Illinois Public Aid Code. Prohibits the Department of Human Services and the Department of Healthcare and Family Services from allowing any recipient of cash assistance or medical assistance provided under the Code or any applicant or recipient of benefits provided under the federal Supplemental Nutrition Assistance Program to submit any income self-attestation form or document as proof of income. Requires the Departments to apply for any federal waivers or approvals necessary to implement the amendatory Act. |

| 2 | | Be it enacted by the People of the State of Illinois, |
| 3 | | represented in the General Assembly:
|
| 4 | | Section 5. The Illinois Public Aid Code is amended by |
| 5 | | adding Section 12-4.60 as follows:
|
| 6 | | (305 ILCS 5/12-4.60 new) |
| 7 | | Sec. 12-4.60. Proof of income. Subject to federal |
| 8 | | approval, if required, the Department of Human Services and |
| 9 | | the Department of Healthcare and Family Services shall not |
| 10 | | allow any applicant or recipient of cash assistance under |
| 11 | | Article III, IV, or XVI, medical assistance under Article V, |
| 12 | | or benefits under the federal Supplemental Nutrition |
| 13 | | Assistance Program to submit any income self-attestation form |
| 14 | | or document as proof of income. The Departments shall apply |
| 15 | | for any federal waivers or approvals necessary to implement |
| 16 | | this Section. |
