 1                    AMENDMENT TO HOUSE BILL 2369

 2        AMENDMENT NO.     .  Amend House Bill 2369 on page 1,  by
 3    replacing lines 12 through 14 with the following:
 4    "by  filing the request on forms prescribed by the Department
 5    and paying a nonrefundable fee as established  by  rule.   If
 6    the  requirements of this Section have been met, all fees for
 7    the  biennial  license  renewal  shall  be  waived   by   the
 8    Department".
