| AMENDMENT NO. ______. Amend House Bill 4712 on page 1, line |

| 17 by inserting after "entity" the following: |

| "; however, a person or entity that provides an insurance card |

| must print on the card an identification number unique to the |
| 6 |
| holder of the card in the format prescribed by Section 15 of |
| 7 |
| the
Uniform Prescription Drug Information Card Act"; and

| on page 3, line 13 by changing "2005" to "2006".
