| AMENDMENT NO. ______. Amend House Bill 4799 on page 1, by |

| replacing lines 7 through 13 with the following:

| "Sec. 356z.8. Multiple sclerosis; physical therapy. A |
| 5 |
| group or individual policy of accident and health insurance or |
| 6 |
| managed care plan amended, delivered, issued, or renewed after |
| 7 |
| the effective date of this amendatory Act of the 94th General |
| 8 |
| Assembly must provide coverage for medically necessary |
| 9 |
| physical therapy as defined by a prescribing physician for |
| 10 |
| insureds diagnosed with multiple sclerosis to achieve and |
| 11 |
| preserve optimal functional status or to reduce the rate of |
| 12 |
| deterioration of the medical condition or body part due to the |
| 13 |
| disease.".
