| WHEREAS, A Silver Pastoral Anniversary Celebration and a |

| Street Naming Ceremony in honor of Bishop Horace and Sister |
| 4 |
| Susan Smith was held on March 19 and 20, 2005, at Apostolic |
| 5 |
| Faith Church in Chicago; and
|
| 6 |
| WHEREAS, Dr. Horace E. Smith received his formal training |
| 7 |
| in Chicago; he graduated an honor student from the Lindblom |
| 8 |
| High School and continued his education, receiving the |
| 9 |
| following degrees: Bachelor of Science (with honors), Chicago |
| 10 |
| State University (1971); M.D., the University of Illinois |
| 11 |
| Medical Center (1975); completion of Pediatric Residency, |
| 12 |
| University of Illinois Hospital (1978); and completion of |
| 13 |
| Clinical Fellowship in Pediatric Hematology/Oncology, |
| 14 |
| Children's Memorial Hospital/Northwestern University (1980); |
| 15 |
| and
|
| 16 |
| WHEREAS, Dr. Smith is director of the Comprehensive Sickle |
| 17 |
| Cell/Thalassemia Program at Children's Memorial Hospital and |
| 18 |
| is recognized world-wide as an outstanding pediatrician in his |
| 19 |
| field, much sought after for his expertise by medical |
| 20 |
| professionals; and |
| 21 |
| WHEREAS, Dr. Smith serves on the Board of the historic |
| 22 |
| Wabash YMCA, which was purchased by four community churches, |
| 23 |
| including Apostolic Faith Church; the churches redeveloped and |
| 24 |
| reshaped the Y for community services, including SRO housing; |
| 25 |
| since 2001, Dr. Smith has also served as a Board Member of The |
| 26 |
| Family Institute at Northwestern University and as a member of |
| 27 |
| the University of Chicago Cancer Research Center-Community |
| 28 |
| Health Advisory Board; and
|
| 29 |
| WHEREAS, In 2002, Bishop Smith and the Apostolic Faith |
| 30 |
| Church partnered with World Vision to sponsor orphaned children |
| 31 |
| whose parents died as a result of HIV/AIDS; to date, |
