AMENDMENT TO SENATE BILL 2295

    AMENDMENT NO. ______. Amend Senate Bill 2295  on page 1,

line 5, by deleting "8-2005,"; and

on page 1, immediately below line 15, by inserting the 
5
following:

    "In this Part, "practitioner" means any health care 
7
practitioner, including a physician, dentist, podiatrist, 
8
advanced practice nurse, physician assistant, clinical 
9
psychologist, or clinical social worker. The term includes a 
10
medical office, health care clinic, health department, group 
11
practice, and any other organizational structure for a licensed 
12
professional to provide health care services. The term does not 
13
include a health care facility as defined in this Section.";

and

on page 3, by replacing lines 7 through 15 with the following:
16
    "Sec. 8-2003. Records of health care practitioners. In this 
17
Section, "practitioner" means any health care practitioner, 
18
including a physician, dentist, podiatrist, advanced practice 
19
nurse, physician assistant, clinical psychologist, or clinical 
20
social worker. The term includes a medical office, health care 
21
clinic, health department, group practice, and any other 
22
organizational structure for a licensed professional to 
23
provide health care services. The term does not include a 
24
health care facility as defined in Section 8 2001."; and
