AMENDMENT TO SENATE BILL 2695

    AMENDMENT NO. ______. Amend Senate Bill 2695 on page 1,

line 11 by inserting after the period the following:

"With the consent of the resident or his or her representative, 
5
the facility must inform the resident's designated case 
6
coordination unit, as defined in 89 Ill. Adm. Code 240.260, of 
7
the resident's pending discharge and must provide the resident 
8
or his or her representative with the case coordination unit's 
9
telephone number and other contact information."; and
 
10
on page 1, by replacing lines 17 through 23 with the following:
11
"removal under Section 3-422.".
