HOUSE RESOLUTION

WHEREAS, The members of the Illinois House of Representatives are saddened to learn of the death of David M. Hayes of Roseville, who passed away on August 23, 2007; and

WHEREAS, He was born on October 6, 1961 in Monmouth, the son of Gary and Donna Smith Hayes; he married Karen Grant on July 13, 1985 in Monmouth; and

WHEREAS, David Hayes attended the Roseville schools; he worked as an Emergency Medical Technician for United Ambulance; he worked for the Corman Memorial Homes; and he was a paramedic for Galesburg Hospital Ambulance; and

WHEREAS, He was a member and past president of Roseville Ambulance Service, a member of the Roseville Fire Department, and District Fire Marshall for the R.S.P. & E. Fire Protection District; and he was Assistant Chief of the Roseville Rescue Squad; and

WHEREAS, David Hayes was a member and past Master of the Roseville Masonic Lodge #519 and a member of the Scottish Rite Bodies, Valley of Peoria; he was a Shriner for Mohammed Shrine of Peoria, a member and president of the Roseville Township Cemetery and of the Spark Plug Collectors; he was a member of the Swan Creek United Methodist Church and a member of the Warren County 911 board; and

WHEREAS, He was preceded in death by his maternal grandparents and his paternal grandfather; and

WHEREAS, The passing of David Hayes is deeply felt by his wife, Karen; his parents, Gary and Donna Hayes; his brother, Jeff Hayes; and his paternal grandmother, Blanche Hayes; therefore, be it

RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-FIFTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn, along with his family and friends, the passing of David M. Hayes; and be it further

RESOLVED, That a suitable copy of this resolution be presented to the family of David M. Hayes as a symbol of our sincere sympathy.
