| AN ACT concerning State government.
|

| Be it enacted by the People of the State of Illinois,
|

| represented in the General Assembly:
|

| Section 5. The Illinois Act on the Aging is amended by |
| 5 |
| adding Section 4.08 as follows: |
| 6 |
| (20 ILCS 105/4.08 new) |
| 7 |
| Sec. 4.08. Medication management program. Subject to |
| 8 |
| appropriation, the Department shall establish a program to |
| 9 |
| assist persons 60 years of age or older in managing their |
| 10 |
| medications through agreements with local case coordination |
| 11 |
| units. The case coordination units may contract with various |
| 12 |
| community agencies or programs for medication management |
| 13 |
| services, including but not limited to home health agencies, |
| 14 |
| facilities licensed under the Nursing Home Care Act or the |
| 15 |
| Assisted Living and Shared Housing Act, public health |
| 16 |
| departments, or hospitals. The Department shall establish |
| 17 |
| guidelines and standards for the program by rule. The program |
| 18 |
| may include but not be limited to the use of a licensed health |
| 19 |
| care professional to provide such services as medication |
| 20 |
| reminders, medication monitoring, medication set-up, and |
| 21 |
| storage of medication. The licensed health care professional |
| 22 |
| may also interface with the client's caregivers, pharmacist, |
| 23 |
| and physicians. For the purposes of this Section, "licensed |
