    AMENDMENT NO. ______. Amend House Bill 2481 as follows:

on page 2, line 19, by replacing "Physicians" with "Physicians 
4
licensed to practice medicine in all its branches"; and

on page 3, line 13, by replacing "informed" with "written 
6
informed"; and
 
7
on page 5, line 9, immediately after "Section.", by inserting 
8
"No duty to report under this Section exists if the patient's 
9
legal representative refuses written informed consent to 
10
report.".
