| AMENDMENT NO. ______. Amend House Bill 2481 as follows:

| on page 2, line 19, by replacing "Physicians" with "Physicians |

| licensed to practice medicine in all its branches"; and
|

| on page 3, line 13, by replacing "informed" with "written |
| 6 |
| informed"; and
|
| 7 |
| on page 5, line 9, immediately after "Section.", by inserting |
| 8 |
| "No duty to report under this Section exists if the patient's |
| 9 |
| legal representative refuses written informed consent to |
| 10 |
| report.".
