| AMENDMENT NO. ______. Amend House Bill 3641 by replacing |

| everything after the enacting clause with the following:

| "Section 5. The Illinois Public Aid Code is amended by |
| 5 |
| adding Section 12-4.37 as follows:
|

| (305 ILCS 5/12-4.37 new) |

| Sec. 12-4.37. Children's Healthcare Partnership Pilot |
| 8 |
| Program. |
| 9 |
| (a) The Department of Healthcare and Family Services, in |
| 10 |
| cooperation with the Department of Human Services, shall |
| 11 |
| establish a Children's Healthcare Partnership Pilot Program in |
| 12 |
| Sangamon County to fund the provision of various health care |
| 13 |
| services by a single provider, or a group of providers that |
| 14 |
| have entered into an agreement for that purpose, at a single |
| 15 |
| location in the county. Services covered under the pilot |
| 16 |
| program shall include, but need not be limited to, family |

| practice, pediatric, nursing (including advanced practice |
| 2 |
| nursing), psychiatric, dental, and vision services. The |
| 3 |
| Departments shall fund the provision of all services provided |
| 4 |
| under the pilot program using a rate structure that is |
| 5 |
| cost-based and similar to the rate structure used in the case |
| 6 |
| of services provided by a Federally Qualified Health Center. To |
| 7 |
| be selected by the Departments as the provider of health care |
| 8 |
| services under the pilot program, a provider or group of |
| 9 |
| providers must serve a disproportionate share of low-income or |
| 10 |
| indigent patients, including recipients of medical assistance |
| 11 |
| under Article V of this Code. The Departments shall adopt rules |
| 12 |
| as necessary to implement this Section. |
| 13 |
| (b) Implementation of this Section is contingent on federal |
| 14 |
| approval. The Department of Healthcare and Family Services |
| 15 |
| shall take appropriate action by January 1, 2010 to seek |
| 16 |
| federal approval. |
| 17 |
| (c) This Section is inoperative if the provider of health |
| 18 |
| care services under the pilot program receives designation as a |
| 19 |
| Federally Qualified Health Center (FQHC) or FQHC Look-Alike.

| 20 |
| Section 10. The Community Services Act is amended by adding |
| 21 |
| Section 4.6 as follows:
|

| Sec. 4.6. Children's Healthcare Partnership Pilot Program. |

| The Department of Human Services shall participate in the |

| under Section 12-4.37 of the Illinois Public Aid Code and may |

| fund the provision of community services under this Act in |

| Sangamon County through participation in that pilot program.

| Section 15. The Community Support Systems Act is amended by |
| 6 |
| adding Section 3.5 as follows:

| (405 ILCS 35/3.5 new) |
| 8 |
| Sec. 3.5. Children's Healthcare Partnership Pilot Program. |
| 9 |
| The Department of Human Services shall participate in the |
| 10 |
| Children's Healthcare Partnership Pilot Program established |
| 11 |
| under Section 12-4.37 of the Illinois Public Aid Code and may |
| 12 |
| fund the provision of community support system services under |
| 13 |
| this Act in Sangamon County through participation in that pilot |
| 14 |
| program.

| 15 |
| Section 99. Effective date. This Act takes effect upon |
| 16 |
| becoming law.".
