    AMENDMENT NO. ______. Amend House Bill 3715 on page 2, line

6, by inserting after the period the following:

"The parent or legal guardian of the minor seeking an entry of

an order of supervision shall submit to the officer of the 
6
court the following affidavit:

    "I ... (name of parent or legal guardian)  hereby declare

that I am the parent or legal guardian of ... (name of minor 
9
defendant) and hereby acknowledge that I am aware that ...

(name of minor defendant) is requesting a disposition  of court 
11
supervision in this case.  I declare under the penalty of

perjury that the above statements are true and correct.
