    AN ACT concerning elections.

    Be it enacted by the People of the State of Illinois,

represented in the General Assembly:

    Section 5. The Election Code is amended  by changing 
5
Sections 19-3, 20-2, 20-2.1, 20-2.2, 20-2.3, and 20-3 as 
6
follows:

    (10 ILCS 5/19-3)  (from Ch. 46, par. 19-3)

    Sec. 19-3. Application for such ballot shall be made on 
9
blanks to be
furnished by the election authority and 
10
duplication of such application
for ballot is prohibited,

except by the election authority.  The
